IVC Filter/Colon surgery

Fri, March 19, 2010 12:00 AM

Im scheduled to have my colon removed for j-pouch surgery but developed a blood clot in my entire right leg last month.  So, before the surgery, I need to have an IVC filter placed in me to catch the blood clot in case it breaks off.  Has anyone had a filter put in prior to surgery?  Thanks.

Thank you for that.  Im looking forward to the surgery actually (just to have the colon removed and be UC disease free).    :)

I have both CD and clot issues. I had PE and DVT last summer and they put in a filter. It ended up saving my life later, became full of clot and needed to be cleaned out by interventional radiology. I recently had surgery to try and take it out to no avail. Its no problem, the thought was since I don't need it (daily lovenox) it should come out if its simple, the surgeon couldn't get it out easy so it stayed. In my experience there is no need to be overly anxious about the filter. The surg is pretty simple thru the jugular and in my case didn't require general anesthesia.

BTW I just had a coletomy on 3/30 and I'm still in the hospital. All is going relatively well.

I wish you all the best. God Bless.

I work in vascular medicine so i actually see ppl in the hospital who are dx with blood clots. There is a correlation between IBD and blood clots, other risk factors are immobility, recent surgery, hormone replacement tx (birth control pills) or possibly a genetic predisposition to clots. If surgery needs to  be done prior to completion of treatment (which if its considered a situational event is ~3mos) than an IVC filter is recommended. Depending on the filter type they can be removed up to one year (some have shorter removal dates) Typically for IBD patients if they have insurance we lean more toward the injectable blood thinner Lovenox and dont even bother with coumadin (oral) at all. The lovenox can be stopped 24hrs prior to surgery. Most radiologists will remove your filter on blood thinners as the insertion is through a vein. good luck

Hi, thanks for the info.  I think the clot was from medication I had been on for UC (Methotrexate).  I have not had any surgery yet, thats the weird thing.  I think they want to do the filter as a pre-cautionary for while Im off the blood thinner meds.  They say I'll have it in for up to a year.  Probably have it removed right after the 2nd surgery in Aug/Sept.

Weird! While waiting for my J-pouch surgery I got a PE and they never found out where the blood clot originated from, so they put that filter in and gave me blood thinners for three months and had the abdominal surgery, and then right before I went home they took out the IVC filter.  No problemo.  Do you think your blood clot was due to inactivity which in itself was due to the first surgery?  That's what my theory is.
