504 plan at school

I'm currently in the process getting a 504 plan for my eighth grader with chrons. I looked on the Kids and Teens section and read over the 504 template. I feel that as his mother I need to be proactive, and do this before there is a problem. Does anyone else have this for their middle school or high schooler, and is there anything else I should ask for that isn't on the template? Thanks!

I have a 504 plan in place at the school my 16 yr daughter attends. I requested a meeting at the beginning of the year. At that meeting I had the 504 template in hand along with the info. for each and every teacher to read to understand this disease.At that meeting we worked out things to make things easier for my daughter to attend school,  like a private bathroom to use, etc.  I also had a "point person" in place which happens to be my daughter's guidance counselor. When my daughter is out, I call her and she gets all her homework and emails/faxes to me. I also requested the school to provide a laptop computer/and a extra set a book (we keep at home) available for my daughter to have to make things easier for when she misses school due to illness, hospitalizations,etc. The school was great with helping us with her needs! These are some of the things I did. Hope it helps!

i went in before school started this year to find out what i needed to do about putting a plan in place.  the principle talked with me about matt's illness and said she didn't think we needed it, that the teachers could handle it by sending makeup work home.  at the time i thought that sounded good, but matt has missed almost half of the school days so far between feeling bad and all the doctor's appointment.  i think we need a plan in place just to set the stage for next year when he'll go to middle school and have to deal with a different teacher for each class.

Hi!

We're in the process of having a 504 put into place for our son who is a freshman in high school.  Didn't think I would need one, but he's already had an issue with a teacher not wanting to let him make up assignments.  I think it's a very good idea to have one in place.  Our son has Ulcerative Colitis and uses the restroom frequently, especially when stressed.  That was one of the issues I stressed the most.  He also has to snack throughout the day.  I think that is all covered in the template.  The best thing that came out of it was actually speaking with the teachers as a group so that they could understand what he's dealing with.  Many people just don't know about these diseases and need to be educated.  I also took in the pamphlet from this site for "Teachers and School Personnel". The teachers said that it was helpful.

I can't really comment on whether or not it is missing anything but do think the 'sooner the better'.  My support group includes kids who have grown up with IBD and are now freshmen in college.  Their parents explained to me that the sooner the 504 is in place, the better off the kids are with only and annual review/revisit necessary.  And, as school gets more complicated with more teachers, etc, it really protects your child at from losing ground due to the disease.

I think you are doing the right thing.  My daughter is only 6 but we did put one in place and I am just glad there is a 'plan' already.
